Heart @Home offers Compassionate and Educated Caregivers – All Heart @Home caregivers are certified and are the most compassionate, dependable and knowledgeable in the industry. All our caregivers are thoroughly interviewed, screened for vulnerable clearance, reference checked and insured. They also maintain updated CPR and First Aid certification as well as WHMIS and attend ongoing professional development courses.

Dynamic Client-Centered Care Plans – Heart @Home takes a Total Case Management approach to providing person-centered care for all of our clients, including a complimentary initial assessment. Additionally, our case manager will work with you to determine the most appropriate level of care and will select the caregivers to meet your specific needs.

Consistent, Dependable and Professional Home Health Care Services – Heart @Home believes in the value of continuity and consistency of care. Our clients and their families can depend on Heart @Home to provide dependable, punctual, flexible and professional Caregiver(s) for one-on-one home health care.

In addition to our Personal Support in the home, Heart @Home has an additional range of support services to make care a viable, one-stop option.  Those services include safe and convenient transportation to and from doctors’ appointments, hospitals, places of worship, errands and recreational outings, we also offer free home safety Inspections.  We are committed to meeting all care needs, whether in home, hospital or long term care facility, as qualified, caring professionals.
