I am a fully qualified Occupational Therapist licensed to practice in Ontario and registered with The College of Occupational Therapists of Ontario (COTO). I graduated with an Honours B.A. in Psychology from the University of Western Ontario and then graduated from the University of Toronto in Occupational Therapy. I am a founding member of the Independent Occupational Therapists of Ontario (IOTO), a member of Ontario Society of Occupational Therapists (OSOT), International Association of Rehabilitation Professionals (IARP), Life Care Planner Resource Network and the Ontario Rehabilitation Alliance (ORA). I provide services within the auto sector, employer and long-term disability sector, medical legal sector and general community. I specialize in the physical, functional and psychosocial evaluation of individuals who are unable to work or perform the essential tasks of daily living due to illness and injury. I am a Canadian Certified Life Care Planner (CCLCP) with the International Commission on Health Care Certification (ICHCC), having completed a postgraduate course of study in Life Care Planning through the University of Florida. I have experience in completing Loss of Service Analyses for families of individuals who are deceased, as well as the management and determination of future care needs for individuals who have sustained a catastrophic impairment such as spinal cord injury, traumatic brain injury, and multiple traumas. My Life Care Plans and Loss of Service Analyses are used in rehabilitation planning, mediation, arbitration, settlement discussions and litigation. I was designated an Attendant Care DAC Assessor from 2003-2006 and worked in the Employee Rehabilitation Unit at the Scarborough Hospital from January 2001 to January 2002, coordinating modified work programs for hospital employees in conjunction with the Occupational Health Department, performing ergonomic assessments, consulting on purchases of safe hospital equipment and providing hospital-wide education sessions. Currently, I perform assessments for Catastrophic Impairment Designation. I have completed the American Board of Forensic Professionals Impairment Rating course material and have been certified in Impairment and Disability rating based on the AMA Guides to the Evaluation of Permanent Impairment, 4th Edition. I am certified to perform Functional Abilities Evaluations (ARCON). I hold my Advanced Office Ergonomics and Advanced Industrial Ergonomics Certifications and conduct ergonomic assessments and implement appropriate interventions. I obtained my PGAP certification and utilize this therapeutic program to foster an individual's re-engagement in daily activity, life roles and return to the workforce, and to reduce the disability associated with impairment with the ultimate goal of returning to work. I regularly attend seminars and conferences to update my knowledge and practice skills. I also complete Geriatric Legal / Functional Living Assessments which are comprehensive analyses that objectively outline and quantify the older individual’s care needs and provide recommendations that are in the older person’s best interests rather than in the interests of the disputing children or powers of attorney. In order to document the areas of need and concern, determine what is required and make appropriate recommendations, I have acquired knowledge and expertise understanding aging, elder care needs and community resources.
