I have been providing complex case management in private practise for over 22 years and have nearly 40 years working in the medical/nursing field. I am willing to travel outisde the GTA and into under serviced areas to conduct assessments. I am willing conduct E-Based Case Management if the travel distance is excessive in order to reduce costs to the cleint and preserve their funding for rehabilitation services.
